How To Use This Form

Step 1: Fill in your contact information.

Step 2: Select your role as it relates to the Commission.

Step 3: Begin typing the name of your institution or the institution you are submitting a report for. When it appears in the list, select your institution with your mouse.

Step 4: Select the HLC Process such as AQIP, Change, etc. Click here for instructions on which process to select.

Step 5: Once you have selected the HLC Process, the available document types will be displayed. Select the document type.

Step 6: Add a note if needed and then browse to your document. You may rename the document using the box provided, or leave it blank.

Step 7: Copy the letters you see into the CAPTCHA field.

Step 8: Click Save to upload the file.

Instructions for Document Submission

The instructions below detail how to properly submit documents through the Document Upload system. Following these instructions will ensure that documents are submitted correctly and filed into the proper location. Selecting the incorrect HLC process will delay responses from the Commission.

If you have questions or concerns on submitting documents through the Document Upload system, please fill out our help request form.

Important notes:

  • Change requests should be submitted as one document in PDF format, with all attachments included. Please do not submit several documents for one change request.

  • Confirmation of Objectivity and Professional Confidentiality forms should be submitted as one combined document in PDF format.

  • Institution Name will be automatically populated as you type; please use the full name of the institution as listed with the Commission. Institution ID will fill in once the Institution Name is entered.

All documents submitted must be less than 250MB in size. For documents larger than 250MB, please contact Vince Coraci (vcoraci@hlcommission.org) for assistance.

Achieving Accreditation

Institutions: select this option to submit any materials related to Candidacy or Initial Accreditation.

  • Select Self Study Report as the document type for the self study report and any related appendices or supplemental materials for the Biennial Evaluation visit, Candidacy visit, or the Initial Accreditation visit.

Peer reviewers: select this option to submit all Team Reports for Biennial Evaluation visits, Candidacy visits, and Initial Accreditation visits.

To submit your Confirmation of Objectivity and Professional Confidentiality forms, please select Objectivity-Confidentiality under HLC Process.

AQIP

Institutions: select this option to submit the following documents for AQIP events:

  • Federal Compliance
  • Quality Highlights
  • Systems Portfolio

Peer reviewers: select this option to submit the following reports:

  • Comprehensive Quality Review
  • Systems Appraisal

To submit your Confirmation of Objectivity and Professional Confidentiality forms, please select Objectivity-Confidentiality under HLC Process.

Change

Institutions: select this option to submit substantive change requests.

  • Select Change Request for all substantive change requests, including:
    • New Program
    • Courses at a New Degree Level
    • Change in Clock or Credit Hours
    • Branch Campuses and Additional Locations
    • Application for the Notification Program for Additional Locations
    • Teach Out of an Existing Location or Branch Campus
    • Distance Delivery
    • Consortial
    • Contractual
    • Mission or Student Body

Peer reviewers: select this option to submit reports for change requests.

  • For Change Panel Recommendations, select Change Panel Report.
  • For Team Reports after a Change Visit, select Change Visit Report.

To submit your Confirmation of Objectivity and Professional Confidentiality forms, please select Objectivity-Confidentiality under HLC Process.

Inquiries/Other

Institutions:

  • For applications to the Academy for Student Persistence & Completion or Academy for Assessment of Student Learning, select Academy Application.
  • For requests for the Commission to verify accreditation of an institution, program, or location for another agency, select Third Party Requests.
  • For items that do not fall into any of the other categories, select Other.

Peer reviewers: select this option to submit any report that is not listed under Required Reports.

Institutional Responses

Institutions: select this option to submit all Institutional Responses or respond to a Pathways Selection letter you may have received.

Objectivity & Confidentiality

Peer reviewers: select this option to submit your Confirmation of Objectivity and Professional Confidentiality forms.

Quality Initiative

Institutions: select this option to submit any materials associated with the Quality Initiative.

  • Quality Initiative Proposals
  • Quality Initiative Reports
  • Quality Initiative Updates

Note: If you have elected a Commission Sponsored Quality Initiative, check with Commission staff to determine whether you should use this method to submit materials.

Peer reviewers: select this option to submit any materials associated with the review of Quality Initiatives.

  • Quality Initiative Proposal Reviews
  • Quality Initiative Report Reviews
  • Quality Initiative Update Reviews

To submit your Confirmation of Objectivity and Professional Confidentiality forms, please select Objectivity-Confidentiality under HLC Process.

Required Reports

Institutions: select this option to submit required reports requested by the Commission.

  • Select Interim Reports to submit an Interim Report.
  • Select Indicators Report for a Non-Financial Indicators Response or a Financial Recovery Plan.

Peer reviewers: select this option to submit the following reports:

  • Multi-Location Visit reports
  • Multi-Campus Visit reports
  • Campus Evaluation Visit (CEV) reports
  • Focused Visit reports
  • Additional Location Confirmation Visit (ALCV) reports
  • Financial Indicator panel reports
  • Non-Financial Indicator panel reports

If the report does not fall into any of the listed document types, please select Inquiries/Other for the HLC Process.

Seeking Accreditation (Eligibility)

Institutions: select this option to submit any materials related to the application or eligibility processes.

  • Select Preliminary Evidence as the document type for the letter of inquiry and associated preliminary evidence.
  • Select Eligibility Filing for all materials that are required as part of the filing process.
  • Select Letters for any other materials related to the process of seeking accreditation.

Peer reviewers: select this option to submit your Reviewer Analysis for the Eligibility process. Select Reviewer Analysis for the PIF Analysis or the Analysis of the Eligibility Filing.
