Complete this form to determine whether prior HLC approval or notification is required for a contractual arrangement between your institution and an entity not accredited by a recognized accreditor.

Determining the Percentage of Services Provided for the Program

When entering into a contractual arrangement to offer an educational program, there will be services offered by the institution and services offered by the contractual partner. These services may relate to different aspects of the educational activities for the program, which together constitute 100% of the educational program.

The screening form asks institutions to report the percentage of services for the educational program that will be provided by the institution and those that will be provided by the contractual provider. It is the institution’s responsibility to determine these percentages. In doing so, consider the following aspects of the educational activities for the program, as applicable, as well as any other relevant aspects of educational activities:

  • Instruction
  • Oversight of instruction, including establishing academic qualifications for instructional personnel
  • Curriculum design and development, including final authority for curriculum
  • Establishment of academic requirements
  • Assessment of student learning
  • Program administration

Depending on the structure of the educational program, there may also be other aspects of educational activities for which services may be provided by the institution or by the contractual partner.

The U.S. Department of Education has also provided guidance on how to determine the services being provided by each party.

Submit the form separately for each credit-bearing educational program, including degree and certificate programs, being offered through a contractual arrangement.

If the person completing this form is not the institution’s CEO or Accreditation Liaison Officer, it is understood that they have consulted with those individuals and are authorized to submit this form on the institution’s behalf.
